Job Description:
The Dementia Care Coach / Engagement Manager, based in California's
North Bay area, supports the Alzheimer's Association mission to
improve early dementia detection and maximize quality care and
support for persons with dementia and those who care for them. This
position works to secure commitments from healthcare providers
serving persons enrolled in Medi-Cal to participate in the Dementia
Care Aware (DCA) Program. A recently launched collaboration with
the University of California, the DCA Program team recruits,
trains, certifies and supports healthcare providers to conduct a
Cognitive Health Assessment, a new Medi-Cal benefit that identifies
early signs of dementia. The Coach / Manager independently
initiates outreach to clinics, hospitals, physician practices and
other healthcare settings, identifies the best point of contact to
commit to educating their staff about the DCA Program and train key
staff to conduct a Cognitive Health Assessment. This position
collaborates with staff from Alzheimer's Association, the
University of California and others to identify and overcome
barriers through offering or providing problem-solving technical
assistance. Although the position is assigned to our Marin office,
it may work remotely. Regular travel within Northern California is
required.
The DCA Program, including this position, is currently funded for
two years.
Responsibilities: Essential functions and responsibilities include,
but are not limited to:
Independently initiate contact and cultivate relationships with
healthcare professionals within an assigned geographic
areaRepresent the Alzheimer' Association Chapter at in-person and
virtual meetings in a professional mannerEffectively communicate
and consistently follow-up through e-mail and other written
communication, telephone and video and/or in-person meetings to
arrange and make presentations to key decisionmakers at clinics,
hospitals, physician practices and other healthcare
settingsEffectively teach others about and secure commitments for
their participation in the ProgramCollaborate with others to
identify, understand, document and work to overcome barriers to
providers participation in the Program Participate in regularly
scheduled meeting with Program staff and, as needed, meet with
other Alzheimer's Association staffPromote and support all
Alzheimer's Association office activities including community
programs, special events and fundraising efforts
Qualifications: Minimum requirements include:A Bachelor's Degree or
equivalent experienceCompetency in teaching, training, conducting
marketing or sales, delivering educational programs or public
speaking or at least 2 years experience conducting outreach, sales,
marketing or similar activitiesAbility to initiate contact, secure
meetings and communicate in a professional, credible manner with
staff at clinics, hospitals, physician practices and other
healthcare settings Ability to learn about or a demonstrated
understanding of healthcare delivery systems, workflows or
operations in order to identify and communicate with appropriate
decision-makers to accomplish the Project goalsAbility to work
independently in addition to collaboratively and to understand any
personal limitations and ask for help if neededPreferred, not
required: Experience working with or for healthcare providers or
other types of professionals. Knowledge, Skills, and
Abilities:Ability to lead, guide and inspire healthcare providers
to complete training for certification to conduct Cognitive Health
Assessments while demonstrating and promoting open communication,
transparency and teamwork towards the Project goalsAbility to
multi-task, be flexible, and handle changing prioritiesExcellent
written, verbal and interpersonal communication skillsMetric driven
to reach Project goalsAccess to a reliable vehicle, a valid
driver's license and proof of automobile insuranceAbility to travel
days or overnight, as needed, primarily within Northern
California
