  • Full-TimeLocationsShowing 1 locationThe Central Valley PACE (Program of All-Inclusive Care for the Elderly) Director of Center Operations (DCO) is responsible for managing all operational aspects for one regional PACE center and associated Alternative Care Site's (ACS's) served by multiple PACE Interdisciplinary Teams (IDT's) to achieve optimum access to services, excellent customer service, quality, cost, and delivery performance in line with the PACE Mission, strategic goals and values, and serves as the liaison between Center, Support Service Management and leadership. This position will provide support to Center, ACS's and Support Service Managers including Clinic/Day Center, Transportation, Home Care, Therapies, Nutrition, 24/7 Nurse Line, Quality, and Social/Behavioral Health to ensure equitable distribution of resources across multiple IDT's. The DCO will act as the lead executive for Regional Center, ACS's and Support Services, has the responsibility for a wide range of disciplines and services provided across multiple PACE IDT's, including all administrative responsibility related to direct operation of multiple PACE IDT's.Schedule is Monday - Friday, working from 8:00am - 5:00pm.Golden Valley Health Centers offers excellent benefits including Medical: (0 Deductible / $2,000 Individual; $4,000 Family Out-of-Pocket Max), excellent PPO coverages; Dental; Vision; 403(b) with match, FSA plans, gym discounts, and so much more!Essential Duties and Responsibilities
    • Demonstrates effective communication and problem-solving skills.
    • Supports implements and fosters a greater staff understanding of the Mission, Vision and Values of Central Valley PACE.
    • Works in collaboration with PACE leadership and clinical leadership in the development, implementation and evaluation of healthcare services.
    • Achieves access and profitability objectives.
    • Provides direct supervision to Center, ACS and Support Services Managers.
    • Leads employees to accomplish all job objectives; inspires confidence and motivation; clearly defines expectations, and maintains personal effectiveness under pressure.
    • Utilizes data to drive efficiency, quality improvement and staff empowerment.
    • In collaboration with the PACE Senior Director of Operations, develops, monitors and analyzes budgets and financial information and utilizes all resources in an effective and economical manner.
    • Supports a culture of transparency through the development, sharing and on-going updating of data, dashboards, and reports with Center, ACS's and Supportive Services Managers.
    • Ensures equitable distribution of human resources across multiple IDT's that satisfies PACE Participant Care needs.
    • Ensures each center and support service meets regulatory guidelines at both the state and federal level.
    • Acts as Interim Manager for Centers, ACS's or Supportive Services as needed. This will require training and successful completion of role specific competency.
    • Serves as a liaison between center, ACS's, support service management and PACE Leadership.
    • Observes each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
    • Supports general PACE maintenance activities to assure compliance with patient safety, infection control, quality assurance, mandated reporting, consent, and confidentiality policies/procedures.
    • Demonstrates understanding of age-related characteristics and needs of patients served.
    • Assures compliance with policies addressing safe working conditions; monitors unsafe working conditions; recognizes inappropriate and/or ineffective patient care management, resolves issue/problem and completes written reports.
    • Travel to multiple sites as required.
    • Other projects and duties as assigned.Physical Demands
      • Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.
      • Ability to exert maximum muscle force to lift, push, pull or carry patients.
      • Must be able to hear staff/clients on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
      • Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
      • Must have high manual dexterity.
      • Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
      • Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
      • Subject to unpleasant odors.
      • The noise level in the work environment is usually quiet to moderate, but may at times be noisy and crowded.Education/Experience RequirementsMinimum Qualifications:
        • Excellent social skills, professional demeanor, and leadership.
        • Ability to proceed on own initiative using independent judgment and discretion.
        • Proficient in MS Word & Excel.
        • Ability to motivate others.
        • Work with minimal supervision.
        • Meet deadlines and prioritize tasks.
        • Effectively interact with operational and clinical personnel.
        • Collect, correlate, analyze data elements, develop plans, prepare presentations, manage work, and is able to problem solve.
        • Excellent customer service skills, demonstrated ability to build effective relationships with co-workers, supervisors and leaders.
        • Ability to communicate skillfully, handle multiple priorities, maintain a warm and friendly demeanor, perform nursing duties, be flexible, innovative, focus on results and travel frequently or as required.
        • Skilled in teamwork, excellent communication, data entry, computer applications and electronic health records.
        • Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently.
        • Valid California driver's license, vehicle insurance, reliable transportation and acceptable driving record.Education/Experience:
          • Graduate of an accredited school of professional nursing.
          • Current unencumbered CA Registered Nurse (RN) License.
          • Current BLS CPR Card certified by American Heart Association.
          • The DCO RN must have one of the following qualifications:
            • Master's degree, preferred;
            • A registered nurse with a baccalaureate or higher degree in nursing or other health related field with three (3) years of experience within the last five (5) years in a home health agency, primary care clinic or health facility;
            • A registered nurse with four (4) years nursing experience within the last five (5) years in a home health agency, primary care clinic or health facility;
