Job Description:
Position Overview:PRIMARY PURPOSE: -Work in the Quality
Improvement department with -physicians, administration and
hospital staff in initiating and establishing quality improvement
in Sepsis. - Assist in creating protocols for evaluation and
management of Sepsis -patients, and act as primary liaison with
floor staff in implementing Sepsis -care policies. - Provide expert
care to patients and families, including sepsis -evaluation,
monitoring of treatment plans and patient and family education.
-Essential Functions: -Collaborates closely with the Director of
Operations, Medical Director, Interventionalists, NP, Nurse
Managers, colleagues and medical staff to identify opportunities
for improving patient care outcomes though participation in
clinical effectiveness.Performs clinical rounds to assess and
creates a plan of are based on identified strengths and needs for
these patients. - Monitors compliance of Sepsis -core measures in
all Sepsis -patients and notifies Director of non-compliance.
Communicates plan with patient, family and physician.Provides
patient and family education regarding disease process and
established treatment plan. - Makes appropriate referrals to
community and hospital resources to meet identified
needs.Reassesses the patient at regular intervals to evaluate
whether goals are being met and/or new needs are present. - Adjusts
the care plan as needed to reflect changes over time.Serves as
clinical resource and nursing expert to the nursing staff and other
departments in Sepsis -care. - Nursing staff and physicians
involved in the care of Sepsis -patients will participate in annual
educational activities in order to stay current in the management
of Sepsis.Assesses, develops, implements and evaluates education
programs relating to Sepsis -care and programs of general interest
to hospital staff and the community.Develops, implements and
evaluates policies and procedures which reflect current research in
Sepsis care.Participates in Sepsis -Program clinical research
trials.Responds to Sepsis -calls during working hours. - Assists
with nursing management of Sepsis -patient.Accountable for
populating the Sepsis -database and for utilizing this data to
improve the fiscal, quality and clinical outcomes of care related
to Sepsis. Reports data to Joint Commission and Get With The
Guidelines.Serves as primary lead for Sepsis Quality
Improvement.Qualifications:Qualifications: - -Current California
Registered Nurse license required. - -2-3 years experience in acute
care prefer ED, ICU, or Med/Surg. - Clinical RN Coordinator -will
meet Sepsis calls/alerts, rounding on floors, and quality
inmprovements in Sepsis -Requires some experience in developing
educational programs related to Sepsis -maintenance and disease
prevention. Must have knowledge of education and teaching
principles. -Experience in acute care-ED, ICU, Med/Surg care
preferred. - -Clinical Coordinator and research highly desirable. -
-Background in Sepsis disease helpful. - - -Organization:California
