Job Description:
We are seeking an attentive and trustworthy caregiver to join
our staff in providing excellent care to our residents. The
caregiver will plan and follow a daily care schedule with clients,
modify care plans as prescribed by medical professionals, and
report on any new conditions or needs that may arise.
To succeed as a caregiver you should be patient, empathetic, and
have strong people skills. You should be observant and committed to
making your client feel safe and comfortable.
Provides residents with medications services per the health care
orders in regard to administration or assistance with medications
and treatments, per State regulation, also provides direct care to
residents following their individualized service plans.

Responsibilities of the Medication Aide:
1.-------- Resident charts. Keeping documentation current
(Community forms, licensing documentation, physician orders,
incident reports. etc.)
2.-------- Communicate resident status changes.
3.-------- Ensure all medication documentation is current and
correct, including medication administration forms, physician
orders, change of dosages, written orders to confirm telephone
orders, etc.
4.-------- Ensure medication room is completely stocked with all
required continuous, PRN, over-the- Counter (OTC), and other
medications as ordered by the physician.
5.-------- Coordinate medication orders and deliveries with
pharmacies.
6.-------- Follows proper procedure pertaining to incidents/
accidents to the residents.
7.-------- Communicate with physicians and other healthcare
providers as needed.
8.-------- Monitor Psychotropic med use is congruent with physician
orders and ensuring resident behaviors actually warrant the use of
medication.
9.-------- Completes an accurate accounting of all medications,
including controlled substances per policy.
10.-- Control medication room access and key assignment.
11.-- Pour, pass, and assist with administration of medications in
accordance with state regulations.
12.-- Coordinate physician and other medical appointments.
13.-- Read all communication notes regarding the community between
the Caregiver shifts.
14.-- Other duties as assigned.

Qualifications:
To perform this job successfully, an individual must be able to

--Education and Experience:
High school diploma or (GED); and 6 months to 1-year related
experience preferred and/or training in direct care services with
older adults, or equivalent combination of education and
experience. Knowledge of dementia and Alzheimer's is a plus, may
also require special certification per state regulations.
----Special Licenses and other Special Requirements:
Must complete and pass State required Medication Aide/Tech
course.

Standing, walking, sitting, use of hands or fingers to feel, reach
with hands and arms, climb or balance, stoop, kneel, crouch or
crawl, talk, vision or hear. Taste smell and the ability to lift up
to 50 lbs. Requires interaction with co-workers, residents or
vendors. Occasional weekend, evening or night work if needed to
ensure shift coverage. On call basis as needed maybe required from
time to time. Possible exposure to communicable diseases. Potential
injury from transferring, repositioning or lifting residents.
Exposure to latex. Possible exposure to bloodborne pathogens,
exposure to various drugs, chemical, infectious or biological
