programFlexible schedulingWhat Will you be Tasked With?Conducts an
initial evaluation/assessment and report findings to the physician
and Clinical Manager and develops the plan of care promptly for
patients.Performs physical therapy procedures, measurements,
evaluations and diagnostic tests on patients in the home for whom
they have been medically prescribed to aid in establishing or
revising specifics of treatment program and communicates with the
Health Care Provider appropriately.Documents initial evaluation and
treatment plan and re-evaluations of patient's progress every
thirty (30) days and updates plan of care as necessary. Completes
comprehensive assessments and OASIS data, as indicated on a timely
basis.Demonstrates thorough knowledge of the principles and
practices of physical therapy and provides physical therapy
services in accordance with the Health Care Provider's plan of
care.Assists with orientation, provides supervision and
consultation services to Physical Therapy Assistants and documents
assignments, instructions and supervision of Physical Therapy
Assistants effectively and updates assignments as needed.Observes,
records and reports to Health Care Provider the patient's
treatment, response and progress and promptly reports significant
changes and obtains orders to revise plan of care, and documents
accordingly.Communicates with Health Care Provider and other
members of the health care team for the delivery and coordination
of quality physical therapy services.Plans for coordinated services
for patients under care by consulting and working with other health
care team members effectively, and communicating schedules as
assigned.Translates all exercises to patient/CG/staff into
functional activities or activities of daily living.Teaches
appropriate treatment, procedures, safety measures, and supportive
activities in writing, when necessary, to assure effective and safe
follow through.Helps the patient and family accept responsibility
for providing care when necessary and provides information, support
and encouragement to help motivate them in their
progress.Communicates regularly with designated RN and other health
care team members to coordinate services, evaluate patient's
progress and update the plan of care.Helps patients and families to
accept and adjust positively to physical, mental, and social
limitations and refers patients to social services when
necessary.Evaluates patient home environment and incorporates
training safety needs into treatment plan.Completes clear, concise
and accurate clinical notes, which document skilled services
appropriately, and submits all requested documentations on a timely
basis (24 hours).Participates in orientation, case conferences,
in-services and staff meetings are requested.May train the patient
in the use of prosthetic device(s).Assists patients to obtain
needed DME/supplies in accordance with the Health Care Provider's
plan of care and recommends safety modifications as indicated.Uses
proper infection prevention and control procedures, universal
precautions and safety measures.Serves as a member of the
Professional Advisory Committee and participates in quality
performance/improvement activities as requested.Adheres to
patient's policies and procedures.Provides and documents services
in accordance with applicable State, Federal, Medicare, Medical
regulations and standards and third party payer
guidelines.Environmental Conditions and Physical
RequirementsDriving or riding in a motor vehicle, standing,
sitting, walking, bending, reaching and stretching.Our Focus Home

Requirements:Master's degree in Physical TherapyActive Physical
Therapy license in the state of practiceAbility to practice
physical therapy unencumbered within state and federal guidelinesAt
least one year of experience working as a Physical TherapistMust
have a vehicle, current driver's license and appropriate automobile
insuranceAbility to make home visits and provide physical therapy
evaluations and treatments in the home environment, complete
documentation effectively, and submit in a timely mannerPreferred
Requirements:Home health experienceHome Care Home Base
