*AL01**Job Overview*We are seeking a compassionate and dedicated Physical Therapist to join our team at Redwoods Post Acute, a skilled nursing facility in San Jose, CA. This position offers an opportunity to provide high-quality rehabilitative care to a diverse group of patients, including those recovering from surgeries, injuries, or managing chronic conditions. As a Physical Therapist, you will be a vital part of the rehabilitation team, working closely with other healthcare professionals to deliver personalized care that supports patients recovery and mobility goals.*Duties** Assess and evaluate patients' physical therapy needs through detailed assessments and physical examinations.* Develop and implement individualized treatment plans based on patient needs, goals, and medical histories.* Administer various therapeutic interventions, including exercises, manual therapy, modalities, and functional training to improve strength, range of motion, and mobility.* Collaborate with the interdisciplinary healthcare team to coordinate and improve patient care.* Educate patients and their families on rehabilitation techniques, injury prevention, and strategies to enhance mobility and independence.* Participate in case reviews, team meetings, and continuing education to stay current on best practices in physical therapy.* Document patient progress, treatments, and outcomes in compliance with facility policies and legal requirements.*Requirements** Valid Physical Therapist (PT) license in the state of California.* Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.* Previous experience in a skilled nursing facility (SNF) or similar healthcare setting is preferred.* Strong clinical judgment, attention to detail, and excellent communication skills.* Knowledge of current physical therapy trends, techniques, and equipment.* Compassionate and patient-centered approach to care.Job Type: ContractPay: $2,358.00 per weekMedical Specialty:* Orthopedics* Pediatrics* Physical & Rehabilitation MedicineSchedule:* Day shift* Monday to FridayAbility to Commute:* San Jose, CA (Required)Ability to Relocate:* San Jose, CA: Relocate before starting work (Required)Work Location: In personby Jobble
