Description This position will be working 16 hours a week on Sunday and Monday, and will be working out of the Santa Rosa Memorial Hospital. This role implements programs to inform clients of St. Joseph Health, Home Care Services (HCS) capabilities and benefits. Coordinates home health services after patients are referred to assist in the transition from hospital to home and ensure continuity of care between hospital or clinic setting and home. • Education sufficient for licensure.
• California Registered Nurse License upon hire.
• National Provider BLS - American Heart Association within 30 days of hire.
• 1 year of Registered Nurse experience. Preferred qualifications:
