Manager RN Location: Greenbrae, CA 94904 Schedule: Monday through
Friday, 11:00 AM – 7:00 PM (No weekends) Status: Must Be Currently
Active Position Overview: The Case Manager Registered Nurse (RN) is
responsible for coordinating and managing patient care across the
continuum to ensure high-quality, cost-effective outcomes. This
role involves collaboration with interdisciplinary teams, patients,
and families to facilitate efficient transitions of care and
promote optimal recovery. Key Responsibilities: Assess, plan,
implement, and evaluate individualized care plans for assigned
patients. Coordinate patient discharge planning and post-acute care
arrangements. Work collaboratively with physicians, nurses, and
other healthcare professionals to ensure continuity of care.
Monitor utilization of healthcare resources to promote efficiency
and cost-effectiveness. Serve as a patient advocate by supporting
informed decision-making and ensuring access to necessary services.
Maintain documentation in accordance with hospital and regulatory

California Registered Nurse (RN) license. Minimum of 2 years of
recent clinical experience, preferably in case management,
utilization review, or discharge planning. Knowledge of healthcare
regulations, payer systems, and community resources. Strong
