Job Description LanceSoft is seeking a travel nurse RN Home Health for a travel nursing job in Clear Lake, California. Job Description & Requirements - Specialty: Home Health - Discipline: RN - Start Date: 04/27/2026 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel RN Care Manager. Day Shift, 40 hours per week, coverage area is Lake County, CA; mileage (government rate) is in addition as travel is needed to see patients. THIS IS FOR HOME HEALTH NOT HOSPITAL. Home Health experience is A MUST/REQUIRED. MUST have HCHB software experience is required. Must have OASIS experience. Must have 2 years experience. Must have recent experience in specialty. Bachelor's Degree in Nursing (BSN): Preferred. Experience in a care management role: Required. Registered Nurse (RN) licensure in the state of practice: Required. Case management certification: Preferred. AHA Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required. When submitting a profile, please make sure to include: RTO, Birthdate and Social.
