patient, self motivated individuals, must drive and have a vehicle

limited to:Domestic Home Care including, but not limited to

as budgeting, assisting client with good money choicesPromote
healthy choices with diet, medical appointments, medications,
exercise programsIdentify household needs and safety issuesPromote
recreation through social activities, classes, and areas of
personal interestSupport relationships with family and friends, and
facilitation of new relationshipsProvide transportation for school,
work, appointments, and community outingsAttend monthly staff
meetings, in-services, safety meetings, training.Document daily log
notes, medication sheets, incident reports, etc.Communicate any
major changes in client's schedule (hospital visit, taking client
out of town, encountering difficulties in the community, etc.) to
your Supervisor immediatelyRecord actual hours worked on our time
and attendance system via smartphone, tablet, or computer and turn
in on time.Follow Emergency Procedures (chain of authority,
911)Actively participate in CACS fundraising effortsAssuming other
duties, responsibilities and special projects as
neededRequirementsQUALIFICATIONSHigh School Diploma (or GED or High
School Equivalence Certificate) preferredExperience with people
with Intellectual and Developmental Disabilities (IDD)Valid
Driver's LicensePass background checkA vehicle in documented good
working orderUp to date CPR certificationSalary Description Hourly,
