Position Title: On-call Substitute Worker Program(s) and Location:
Crisis Respite Center, Palmer Apartments, Outreach & Engagement
Center (Sacramento, CA) Schedule : On-Call, as-needed basis
covering various shift schedules (may require working day, evening,
overnight, and weekend schedules), applicants would need to be
flexible with the varying schedules that require coverage Pay rate:
$19.96/hour About the Position: In our residential programs, the
Substitute Worker has the responsibility of working closely with
residents who are progressing toward independent living. The
Substitute Worker provides the daily support needed for each
resident to function at the highest possible level. These supports
will include emotional support, demonstrating and teaching living
skills, coordinating program activities, and facilitating groups,
and client activities. At our Crisis Respite Center (CRC), the
Substitute Worker will assist individuals in managing their crisis
and will offer information, advocacy, and referrals for continued
assistance and support after they leave the CRC. The (CRC) is a
program designed to provide any Sacramento / Sacramento County
resident (18 years and older) who is experiencing a mental health
hardship/crisis a place to go to receive support and gain hope for
a resolution. The CRC is a person-centered program, providing a
welcoming and non-judgmental environment for individuals who need
relief from the stresses of life, and who need assistance in
stabilizing their mental health crisis. The individuals who come to
the CRC may be experiencing a wide variety of mental health issues.
Please note, this position is on an as-needed basis, there is no
guaranteed hours and/or shifts. Essential Duties &
Responsibilities: Support individuals with active listening,
demonstration of empathy and carry a message of hope and recovery
Observe, assess, intervene and refer (within one’s scope of
practice) in situations with clients regarding mental health
symptoms, physical health symptoms and substance use issues Develop
service/goal plans with clients and meet with the clients as needed
to encourage, support and advocate for progress and assist with
skill development Provide positive public relations, community
referrals, information and support by linking clients to needed
resources; advocate with and on behalf of clients and significant
support persons the clients have identified Accurately complete all
necessary forms, reports and paperwork required by the program on a
timely basis (e.g., progress notes, special incident reports,
discharge summaries, Medi-Cal billing, Avatar forms, HMIS) Shop for
food and household supplies to meet program or client needs Ensure
the office, house, rooms, units and yard are clean and fit for
occupancy, where applicable. Provide and assist with cleaning as
needed Act as a designated Mandated Reporter for the observed or
suspected abuse and/or neglect of children, disabled individuals,
dependent adults and elder/senior citizens. Mandated Reporter also
includes “Duty to Warn” which includes the responsibility to notify
authorities and the intended victim(s) of physical harm Ensure the
protection and confidentiality of all Protected Health Information
Qualifications: Ability to work with individuals who have a mental
health diagnosis. Experience is highly desirable Lived experience
as a mental health consumer or a family member/loved one of a
mental health consumer is desirable Knowledge of Sacramento
community organizations and community resources is highly desirable
Mental health crisis intervention experience is highly desirable
Experience with persons who have substance abuse problems,
preferably individuals who are dually diagnosed (mental health &
substance abuse) is highly desirable Knowledge of 12-Step Programs
and/or personal experience with substance abuse recovery is highly
desirable Other Skills, Abilities, and Job Requirements: Ability to
pass clearance of the Federal Bureau of Investigation (FBI) and/or
Department of Justice (DOJ) background screenings Consumer of
mental health services or a family member of a mental health
consumer is desirable Benefits: 40 hours paid sick time HOPE
